Registrations till Sep 2020: 23,980

Wayand District saw its first cases of COVID-19 in Feb 2020. Since then a combination of fear of infection, and a series of lockdowns has resulted in patients turning up at the health facilities only for emergencies. Also, the directives from the State confined routine outpatients to their homes. In addition, delivery of drugs to the houses of the patients were not, and are still not documented.

Registrations across Facility Type: Types = SDH, FHC & PHC

The reason for lesser registrations at the SDH and CHC is that these are converted into COVID-19 First Line Treatment Centers or full-fledged COVID-19 designated centers, and so no NCD clinics are held presently in these facilities.

However, a new advisory is expected from the State on restarting NCD program without putting the geriatric population at risk.

Treatment Analysis: (cards=4063)

Though Missed visits, and Door step delivery of drugs are shown separately, some facilities are marking them together. Hence this break up is not accurate, though their totals are.

Supportive Supervision: (n=36 Facilities)

Details No. Percent

No. of facilities with risk of stock-outs: n=37 Facilities

Low-risk
<90 days
Amlodipine 0 0 0 0
Telmisartan+Losartan 0 0 0 0
HCTZ+Clorthalidone 7 3 5 6

Most of the hospitals having stock out for Chlorthalidone do not have patients who require Chlorthalidone or diuretics. Also, borrowing between facilities within the district has helped to tide over all deficits so far. Credit for monitoring this IHCI protocol drug distribution system without stock outs goes to the Warehouse Manager.

Key Achievements & Recommendations

  1. Documenting the NCD drugs that were distributed to the patients at their houses, will reduce the number of Missed Visits.
  2. Restarting the Patient Support Groups (PSG) with the Palliative Care Team instead of ASHAs as ASHAs are now deputed for COVID-19 duties.
  3. State Advisory on restarting NCD program without putting the older patients into danger on the anvil.
  4. More Tribal Mobile Medical Units.

High & Low Performers

  • The Annual and Quarterly high and low performing facilities being ascertained from the Annual and Quarterly Review, as most of the facilities cancelled their NCD Clinics due to COVID-19
  • Certificates of Appreciation will be given this year onwards along with usual prizes to the top three facilities with best Hypertension Control Rates in the Annual Review
  • An additional prize would be awarded to the District Hospital, Mananthavady, which has the best control rates once the data from the hospital is got.

Staffing

Facilities having NCD Nurse

Two CHCs are having designated nurse, and they cover only eight hours in a day.

Also, almost all PHCs in the district require a designated NCD nurse.

NCD Nurse: continued

In addition:
Some staff nurses are on COVID-19 duty, and some others on quarantine.

A designated NCD nurse is a must for NCD and IHCI integration in Wayanad
